INSURANCE & REFERRALS |

Some insurance companies require you to obtain a
referral authorization before seeking healthcare from a specialist. If your
insurance company requires a referral, please make an appointment with your
primary care provider to evaluate your needs and together determine your best
plan of treatment.
If you do not receive a referral from your Primary Care Provider prior to an
appointment, your insurance company may not cover your visit.
BE SURE YOU HAVE AN AUTHORIZATION NUMBER FROM YOUR INSURANCE COMPANY BEFORE
GOING TO YOUR APPOINTMENT.
In addition, you may want to call the Customer Service number on the back of
your insurance card. Ask if the facility you have an appointment with is a
"Contracted Provider" and if your benefits cover the type of care you are
receiving. It is not our Facility’s responsibility to contact your insurance to
verify benefits.
