It is the goal of Lewiston Orthopaedic Associates to provide the highest possible quality care in an efficient and cost effective manner. We are happy to assist you with any questions you may have. Insurance information changes rapidly though, so it is hard for anyone to keep up and well informed on the thousands of plans that are available. Knowing your own particular plan’s coverage and understanding your co-pay and deductibles will be helpful to you in determining your out-of-pocket expenses.

Which Insurance companies do you bill?

    We accept and bill most major insurance plans as a regular service to our patients. All we ask is that you present your insurance card at each visit so that we can verify the correct billing for your visit. See our list of insurance's.

Do you take Medicare Assignment?

    All providers here at Lewiston Orthopaedic Associates are participating providers within the Medicare program. We accept and bill Medicare for you as well as your supplemental insurance to Medicare. You are responsible for any deductibles or co-insurance portions after Medicare if you do not have supplemental coverage. What does "contracted" and "allowable" mean?

    A contracted insurance means that we have agreed to the particular insurance company's fee schedule for payment of services. Allowable is just another term used for this. Although we accept the Insurance allowable, there is usually a portion, percentage, deductible or co-pay that the patient is still responsible for.

When is payment expected from the patient?

    We do expect payment at the time of service. If we are on contract with an insurance company, we expect the patient's portion of our fee, co-pay or deductible at the time of service. We accept cash, check, Visa, Discover and MasterCard as payment options.

Do you charge interest?

    No. However we do charge a re-billing fee of $4.00 for accounts over 90 days old.

Civil Suits, Auto, Home or Business Owners Claims

    If you are involved in a civil suit, auto, home or business owner’s accident and are seeking payment from the responsible party, as a courtesy to all our patients, we will submit an insurance claim for you to the responsible party. The patient will be required to submit all necessary information in order to bill the claim to the carrier. Lack of billing information will result in the account being set up as patient responsibility. We do not hold the responsible party liable for your charges incurred due to the length of time it takes to settle these claims; it is the patient’s ultimate responsibility for the balance of the account.

Who is responsible for payment?

    Although we are happy to assist you in as many ways as we can, we ask that you be fully responsible for knowing the specifics of your particular insurance coverage. Examples of these specifics include: Co-pays, deductibles, second opinions, pre-authorizations, preferred providers, referrals, covered and non-covered services and preferred hospitals and facilities.

    If your insurance company requests pre-authorization for your test and/or surgical procedure, our office will initiate the pre-authorization. However, it is the patient’s responsibility to verify pre-authorization has been completed with their insurance company prior to testing/surgery. Please note that pre-authorization does not guarantee payment. Payment is made according to your contract. The clinic cannot accept responsibility for collecting your reimbursement or negotiating a settlement on a disputed claim.

    If you are 18 or over, you are legally responsible for your own account. If the patient is under 18, BOTH parents, despite divorce or separation agreements, OR the legal guardian are responsible for payment.

    All patients under the age of 18 must be accompanied on each visit with a parent or legal guardian. On any patient under the age of 18, if the non-custodial patient is not listed on the HIPAA Consent form, medical information will not be released to the non-custodial patient – regardless of who is responsible for payment of the account.
